Registration Due Dates


June 1 -  Initial $1000 deposit due for all teams to Treasurer See League Payment section for invoice
July 1 -  Any requests for schedule considerations are due to the Scheduler 
July 19 -  Manager Name and contact info due to Registrar
Aug 31 -  Registration paperwork due. Bring to Board meeting or Deliver to Registrar
Dec 15 -   Last day for additions to rosters.
NOTE: this is a date change beginning the 2019-2020 season to ensure all rostered players have adequate time to meet the game eligibility requirements for end of season tournaments. 
Forms Required To Complete Your Team's GNASH Registration

Note:  For returning players/coaches/board members, you will be need to have an access code to complete the registration form.  If you do not know your code, click the link below the access code box, and the code will be emailed to you.


For Board Members/Reps/Alt Reps/Team Managers - Board Member Online Registration Form

  1. USA Hockey Registration confirmation (register as ice manager/volunteer at no cost) – – make sure it is for the CURRENT SEASON - [to be completed online at www.usahockey.com]
  2. Complete SafeSport training Must have current SafeSport certification on file by September 1  (As of 4/1/19, SafeSport Training and the Refresher Course Training are valid for only one (1) season)
  3. Background Check – through USA Hockey -  https://www.usahockey.com/backgroundscreen  (valid for 2 seasons)
From USA Hockey re: Background Checks:  If you are in a position of authority over or have regular contact with USA Hockey athletes, including without limitation a staff member, official, coach, board member, coordinator, local program administrator, trainer, locker room monitor, independent contractor, volunteer, medical personnel, or other person with authority over or have regular contact with USA Hockey athletes, then you are required to complete a background check as a condition of your involvement. 

For Team:

  1. GNASH Academic Eligibility(1 per team) – must list all players and be signed by principal, assistant principal or athletic director.
  2. Team Roster (1 per team) – this must list all players and coaches with numbers and positions.


For Players:  Player Online Registration Form

Files to be upload to online registration:
  1. GNASH Medical Clearance Form (1 per player) - This is replacement for the old Med02 form. This form must be signed by a physician clearing the player to play ice hockey.
  2. Consent to Treat 
  3. Concussion  Parent & Athlete Form & Sudden Cardiac Arrest Form (1 per player) - signed by player and parents - the attachment include both required forms in a single file, signatures are required on page 2 and 5
Other Information Needed:
  1. USA Hockey Registration confirmation page (1 per player) – make sure it is for the CURRENT SEASON - [to be completed online at www.usahockey.com]
  2. Player's Code of Conduct (1 per player) – signed electronically by player and parents via registration form
  3. Parent Clearance-Code of Conduct (1 per player) – This is a statement signed by all parents verifying the player is cleared to play ice hockey, that they are committed to follow the parents code of conduct, and will support the players code of conduct, signed electronically via registration form
  4. SafeSport certification (players age seventeen (17) and older on or before December 31st) - must be completed prior to on-ice or off-ice participation 
  5. Copy of player's birth certificate - This is a one time submission, for any player that is new to GNASH, and has never played travel hockey.  If a player has previously played in GNASH or has played on a travel hockey team, their birth certifcate is on file.


For Coaches: Coach Online Registration Form

Forms to be uploaded to online Registration:
  1. Concussion Coach's Form (1 per coach)
  2. Concussion Course - visit this link http://www.cdc.gov/headsup/youthsports/training/index.html to complete the video course and turn in your certificate. (TN Law)
Other Information Needed
  1. USA Hockey Registration confirmation – make sure it is for the CURRENT SEASON - [to be completed online at www.usahockey.com]
  2. Coach's Code of Conduct signed electronically via online registration
  3. Background Check (1 per coach) – through USA Hockey -  https://www.usahockey.com/backgroundscreen  (valid for 2 seasons)
  4. Must have current SafeSport certification on file by September 1  (As of 4/1/19, SafeSport Training and the Refresher Course Training are valid for only one (1) season)
  5. Meet USA Hockey coach certification progression requirements by 10/15 including age specific module 16/18/19 & under.
